What is SomnoGuard® AP Pro?

SomnoGuard® AP Pro is a dental appliance for snoring and sleep apnoea. The two-part, infinitely adjustable mandibular advancement device is fabricated individually in the dental laboratory. SomnoGuard® AP Pro is the custom-made version of the prefabricated SomnoGuard® AP oral appliance. Note: In the download area you will find studies with the prefabricated SomnoGuard® AP oral appliance.

The fabrication of the dental oral appliance is simple and uncomplicated. After the dental impression has been taken by a dentist, SomnoGuard® AP Pro is fabricated in the dental laboratory using the metal components and the usual procedures and materials. SomnoGuard® AP Pro is thus individually adapted to the patient’s teeth and jaw conditions.

The upper and lower jaw trays are manufactured individually with the help of a so-called pressure moulding technique. The dentist uses an alginate impression and a plaster jaw model. The metal components made of high-quality stainless steel connect the upper and lower tray and form the heart of the new development. The protrusion technology is derived directly from the prefabricated SomnoGuard® AP oral appliance.

How does SomnoGuard® AP Pro work?

The SomnoGuard® AP Pro custom-made mandibular advancement device can reduce or completely prevent snoring and nocturnal breathing stops in obstructive sleep apnoea. The dental appliance moves your lower jaw infinitely forward from 0 mm to approximately 10 mm. Depending on the extent of the adjusted lower jaw advancement, the upper airways are more open so that you can breathe more freely.

How is the lower jaw advancement of SomnoGuard® AP Pro adjusted?

The SomnoGuard® AP Pro is supplied with two locknuts, a lower and upper tray attachement, a spanner, an adjusting tool and three screws of different lengths. In the initial titration phase, the patient can adjust or change the lower jaw advancement between 0 mm and approximately 10 mm using the adjusting tool. The lower jaw advancement depends on the length of the selected screw.

The dentist or dental laboratory fixes the selected protrusion by tightening a locknut on the screw thread in the anterior position before the housing.

Instructions for the fabrication of SomnoGuard® AP Pro

After a construction bite has been taken by a dentist, a plaster model is constructed in the dental laboratory and articulated with the aid of the protrusion bite. After duplicating the model, the pressure moulding technique is used to deep-draw hard-elastic trays for the upper and lower jaw as the construction basis for the custom-made mandibular advancement device. All commercially available thermoforming foils in various thicknesses can be used for the manufacturing process, e.g. Erkoloc Pro (Erkodent GmbH) in a thickness of 2 to 3 mm.

An acrylic adhesive is then applied to the areas in the anterior region where the metal components are to be fixed. The lower tray attachement with the housing is fixed to the underside of the lower jaw tray with acrylic adhesive. The fixing holes of the base plate are covered with crystal clear, cold polymerising acrylic. Acrylic can be applied in the molar area to ensure distal support (if specified by the dentist). Using the same procedure, the transverse C-bar is fixed to the upper tray with acrylic adhesive in the anterior region. After the acrylic adhesive has cured, the trays are ground and polished. The upper and lower jaw trays are then connected to each other with an adjusting screw.

Composition of the metal components

The metal components are made of stainless steel from Thyssen Krupp with the material number 1.4301 (AISI No. 304) according to DIN EN 10088-1. The chemical composition is usually as follows:
C < 0.07 wt.%, Cr 17.0 – 19.5 wt.%, Ni 8.0 – 10.5 wt.% and N < 0.11 wt.%, Si < 1.0 wt.%, Mn < 2.0 wt.%.
